Default Candle light help please :)

Its my Dad's birthday today 56 years old and I really want to get a picture of him lit up by the birthday cake candles.

I have no clue how to set up the camera for this and unfortunately the window of time to get it right is going to be short.

I'm sure the shutter speed will be low, but what else should I look for when taking this picture?? should I use flash at all? I have the tripod and plan on using that, but I was thinking that his face in focus with a blurred candle lit cake would look cool. Maybe just the top of the cake and the candles showing in the foreground??

I'm worried about the noise caused by a high ISO, him being blurry from a long shutter speed, and composition in general. I've seen a few good ones over the years, but when the time comes I will probubly only have a few minutes to set-up.

Any tips would would be really great I'll post the picture here later on tonight to share the results.
