I use the lens cap leashes (w/the elastic band); If i'm shooting a lot, i just pull it off and pocket it, else I just hold it w/ my left hand. This isn't a prob when shooting on a tripod. I got my gf a clip that threads on her camera strap that she can just slip it onto (there is a similar one on photojojo.com); she loves it.

I used to use a piece of velcro on the lenscap and the other velcro on the camera back before we ha LCD screens, Now I stick it on my camera bag.

In my pocket or leave it in the camera bag. I never go out shooting with the cap on.

Mine says in the bag until the camera goes in the bag - less chance of missing a shot. The hood stays on the lens until it goes back in the bag (reversed on the lens when storing).
