I want to enter a photo contest ,but their asking me to get a written consent form ,for the individual in the photo that I want to enter. Where do I get a release form or can I just write something down and get her to sign it ...I know sounds silly lol.

If you don't have one and don't want to afford a lawyer to draw up one for you, go and steal the one from Getty, Corbis or any other stock image agency. Or just go to google and type the magic incantation "model release form template".

there is an app for either Iphone or Droid phones called EASY RELEASE.....it is a model release you can use in the field right from your phone....and it will email the finished release to your account.

This is what I use: Sample Photography Release Form for Photographers Miami Wedding Photography | Destination Wedding Photographers | Key West Photographer | Indian Wedding Photographer

Change the name and you should be fine.

I just started using the Easy Release app on my iPad this past weekend. I really like it and am going to use it exclusively now.

It has some standard releases for both models and property then it allows you to customize them or create your own.

Make sure the individual realizes that they may appear in an advertisement. (Especially if this is a friend)

Yes they sign on the iPad. You take a picture of them and a picture of their ID. And it puts both on the release.
You sign as well. Then you send it to their email in a PDF. I am becoming so much more organized with this.
