So does this look like a real tattoo or a fake? I was downloading tattos from a site where they provide free templates and played around with displacement maps and
different opacities.

Lee, thanks for having a look. You mean it's too faded? How about this?

#1 is 50% opacity and #2 is 60% opacity. Which one looks more realistic? I have a friend who wants a fake tattoo in a portrait.

No idea yet, its acutally a guy and he just had the idea, so I was looking at ways to do this.

Have a look here: some are free and some for a fee (higher resolution templates).

Those free ones are not really of good quality but for testing purposes ok. I just had to adjust with levels to get a clean white background. extraxt the rose from it and paste it to an image with a transparent image. The more complicated thing for me was and still is to get a nice replacement map. Followed this tutorial Painless Tattoos | Planet Photoshop

That looks really good!!

1 thing to help make it look a little more realistic would be to add some noise texture to the tattoo. Also the liquefy filter can help shape it in conjunction with the displacement maps.

But again it looks really good IMO. I think the first image you posted looks the best of the 3

mhh ... seems to be no clear opinion which on is more realistic. Maybe I have to do a real world check and take a photo of a friend who has a real tatto for comparison.
