For anyone interested, I've just done a short tutorial here Macro Exercises #4 Lighting - Digital Grin Photography Forum
on lighting in macro shots- this is just on simple outdoor shooting.

Fraid they are both part homemade. The first is a standard generic vidcam light bracket with a ball head attached to the top. The second is a silly mini tripod I had that I fixed to a straight piece of aluminium.
The nearest I've seen to this sort of design is a Hakuba LH-1 ballhead bracket on Ebay.
